Which BEST describes the pathophysiology of mucous membrane pemphigoid?

  1. Reactive
  2. Immune-mediated (autoimmune)
  3. Inflammatory only
  4. Viral
Show answer and explanation

Correct answer: B. Immune-mediated (autoimmune)

Mucous membrane pemphigoid (MMP) is an autoimmune (immune-mediated) blistering disorder. Auto-antibodies (IgG, IgA) attack hemidesmosomes/basement membrane components (BP180, BP230, laminin-332). This causes subepithelial separation/blistering. It is NOT reactive (reactive lesions have a trigger like trauma/irritation). Not viral. The immune attack on BMZ = autoimmune.
