A patient is on clopidogrel and aspirin. What dental treatment modifications are needed?
Show answer and explanation
Correct answer: A. No modifications needed
- A. Current ADA guidelines support performing most dental procedures WITHOUT stopping aspirin or clopidogrel. The risk of a thromboembolic event (MI or stroke) from stopping medication FAR outweighs the risk of minor increased bleeding that can be managed with local hemostatic measures (pressure, sutures, hemostatic agents).
- B. General anesthesia is not indicated simply because a patient is on antiplatelet therapy. Local hemostatic measures are far safer and more appropriate.
- C. Stopping antiplatelet therapy increases risk of stent thrombosis, MI, or stroke — the risk does NOT outweigh the benefit for routine dental procedures.
- D. INR is used for warfarin monitoring — NOT for antiplatelet therapy. Clopidogrel and aspirin do not affect INR.
