A patient has mucous membrane pemphigoid (MMP). Which specialist should you refer them to?
Show answer and explanation
Correct answer: B. Ophthalmologist
Ophthalmologist (B). Mucous Membrane Pemphigoid (cicatricial pemphigoid) is an autoimmune blistering disease that affects mucous membranes — including eyes. Ocular MMP (conjunctival MMP) can cause progressive scarring of the conjunctiva leading to symblepharon, entropion, and blindness if untreated.
• Ophthalmology referral is MANDATORY for all MMP patients to screen for and treat ocular involvement before irreversible vision loss occurs.
• Other specialists involved: Dermatology (skin lesions), Urology/Gynecology (genital lesions), Otolaryngology (laryngeal lesions causing airway compromise).
• Clinical features: Desquamative gingivitis is the most common oral presentation — gingival erythema, vesicles, erosions. Nikolsky sign often positive. Biopsy + DIF (direct immunofluorescence) for diagnosis.
