Prosthodontics & Occlusion · single question

A patient with a complete denture presents with epulis fissuratum. The denture is relieved (adjusted), but the epulis persists. What is the next step?

  1. Continue wearing the denture and monitor
  2. Surgical excision of the epulis
  3. Make a new complete denture immediately
  4. Prescribe anti-inflammatory medication
Show answer and explanation

Correct answer: B. Surgical excision of the epulis

Epulis fissuratum (denture-induced hyperplasia) is caused by chronic irritation from ill-fitting denture flanges. Initial management is denture relief (adjusting/trimming the overextended flange) and allowing the tissue to resolve. If the hyperplastic tissue does NOT resolve after denture correction, surgical excision is required, followed by fabrication of new dentures. The tissue does not always self-resolve once well-established.
