Medical Emergencies · patient case question
Considering his overall medical status, what is the recommended maximum number of cartridges of 2% lidocaine with 1:100,000 epinephrine for a single appointment?
Patient case: COPD, a Black Tongue, and a Lateral Tongue Mass
Patient: 58-year-old male
Chief Complaint: “My tongue looks black and furry, and there’s a lump on the side of it.”
Background: Chronic obstructive pulmonary disease, GOLD stage III. 40 pack-year smoking history and still smoking one pack daily. Uses a chlorhexidine rinse intermittently. No cardiovascular disease.
Medications: Albuterol inhaler PRN · Tiotropium daily · Inhaled fluticasone
Vital Signs: BP 130/78 mmHg · HR 88 bpm · RR 22 · SpO₂ 92% on room air · T 98.4°F
Findings: Barrel chest, pursed-lip breathing, prolonged expiration. Dorsal tongue with markedly elongated, brown-black filiform papillae. A separate firm, nodular, exophytic mass approximately 15 mm across on the left lateral tongue, present for several months and slowly enlarging. Extensive restorative needs across three quadrants.
Show answer and explanation
Correct answer: A. 2 cartridges
- A. Correct. For the medically compromised patient the accepted epinephrine ceiling is 0.04 mg per appointment. Each cartridge of 1:100,000 contains 0.018 mg, so two cartridges deliver 0.036 mg and a third would exceed the limit.
- B. Incorrect. Five cartridges deliver 0.09 mg. This approaches the healthy-adult range but is well beyond a restricted limit.
- C. Incorrect. Four cartridges deliver 0.072 mg, nearly double the restricted ceiling.
- D. Incorrect. Three cartridges deliver 0.054 mg of epinephrine, above the 0.04 mg ceiling applied to medically compromised patients.
Note: Flagged — read this one carefully. The 0.04 mg epinephrine ceiling is defined for significant cardiovascular disease, and COPD alone does not strictly invoke it. This patient is ASA III with hypoxemia and takes a beta-2 agonist, which is why board recalls apply the restricted limit. For a healthy ASA I adult the ceiling is 0.2 mg, roughly 11 cartridges, and the lidocaine dose itself caps out first in practice at about 13 cartridges (500 mg absolute maximum).
