Prosthodontics & Occlusion · single question
A patient who has worn the same maxillary denture for 15 years presents with inflammatory papillary hyperplasia of the palate. Each of the following is an appropriate step EXCEPT:
Show answer and explanation
Correct answer: B. Taking a final impression at this visit
- A. ❌ This is appropriate and is the single most effective measure — continuous denture wear is the primary cause.
- B. ✅ CORRECT — This is the EXCEPTION. A FINAL impression must never be taken over inflamed, hyperplastic tissue. The record would capture distorted anatomy, and the tissue will change shape as it heals. Resolve the inflammation first — tissue rest, antifungals, denture hygiene, possible tissue conditioner — then reassess. Residual hyperplasia may need surgical excision before the definitive impression.
- C. ❌ This is acceptable. A preliminary impression for study purposes and tray fabrication does not commit you to the current tissue form.
- D. ❌ This is appropriate. Candida is almost invariably involved, and the denture itself is the reservoir.
Note: Papillary hyperplasia = cobblestone nodules on the hard palate under the denture base. Denture stomatitis and epulis fissuratum belong to the same 'ill-fitting denture' family.
