Prosthodontics & Occlusion · single question

A patient has only teeth #22–27 remaining (mandibular anterior sextant only). Which prosthodontic finding is most expected?

  1. Flabby (fibrous) ridge in the anterior mandible with bone resorption
  2. Flabby ridge in the posterior areas only
  3. Loss of interarch space (decreased vertical dimension)
  4. Loss of intraarch space
Show answer and explanation

Correct answer: A. Flabby (fibrous) ridge in the anterior mandible with bone resorption

When only mandibular anterior teeth remain opposing a complete maxillary denture, the anterior natural teeth cause continued bone resorption in the anterior mandible while the ridge in edentulous areas resorbs differently. The anterior natural teeth exert excessive occlusal forces on the mandibular anterior residual ridge, causing a flabby (hypermobile fibrous) ridge anteriorly — a classic finding known as combination syndrome (Kelly's syndrome). This also includes loss of posterior alveolar bone height and anterior maxillary bone loss.
